We are here to help you apply for Adult services and supports from DSO Toronto. Who can apply for Services and Supports from DSO Toronto:

  • an individual turning 18 (as young as 16)
  • an individual recently moved to Toronto
  • an individual who has never received services from a developmental service agencies after the age of 18, never had a lead agency, never been on a waitlist as an adult

About DSO Toronto

Surrey Place Centre is the regional contact point for adult developmental services in Toronto.

Surrey Place Centre has been chosen as one of the 9 DSOs in the province of Ontario. These regional contact points are part of our province's plan to modernize Ontario's developmental services system.

DSO Toronto will make it easier for adults with developmental disabilities and their families to apply for services and supports. The service is designed to help people live with more independence and choice by identifying what supports they need to achieve their goals.

There will be no need for people who currently receive service through the system to apply again through the DSO Toronto Region, but for those who do not receive services; DSO Toronto will offer a single point of access for the system.

This means that adults with developmental disabilities coming to the Adult Sector for the first time will have:

  • the ability to access information, confirm eligibility for services and support needs, and link to services and supports;
  • an easy way to apply for developmental services in their community – one place to go to find information, have eligibility confirmed and be assessed for service needs; and
  • less duplication and a fairer, more consistent and transparent application process for everyone

Applying for Services & Supports

To apply for services and supports please contact DSO Toronto staff who will:

  • confirm if you are eligible to receive services and supports
  • support you in filling out an Application Package
  • create a support plan for you
  • find out if services and supports are available
  • help you connect to services and supports

Please note that you do NOT need to apply to DSO Toronto or call if:

  • you have a lead agency
  • you are on a waitlist waiting for services or supports; for example, residential, day supports, case management
  • you are receiving services from agencies; for example, case management, day supports

You are already deemed eligible for all services and do not need a new psychological assessment. You will not lose services due to any change in eligibility criteria.

You do NOT need to call DSO Toronto if you have a lead agency and are receiving services because:

  • you are deemed eligible for all the services and will not need to provide a psychological assessment to be grand parented into DSO Toronto
  • you will not lose services that you have now and your status will not change on the needs lists
  • you will be receiving the application package to determine your support needs in the future
